Summary:

The Patient Care Coordinator plays a vital role in ensuring that the physician's care plan is executed accurately and promptly. This position involves the coordination of referrals with healthcare providers, hospitals, and community services, scheduling follow-up appointments, and addressing patient inquiries regarding access to the healthcare system.

(This is a part-time role that will support the Endocrinology team.)

Key Responsibilities:

Patient Coordination Duties:

  • Timely acquisition of ancillary results for all physician orders.
  • Securing referring provider notes for all referred patients promptly.
  • Documenting and obtaining authorization requirements from insurance providers for scheduled referrals and procedures.
  • Flexibility to work at various Tryon office locations as necessary.
  • Monitoring assigned Athena boxes and other designated areas for coverage.
  • Additional responsibilities as assigned.

Clinic Support Duties:

  • Compiling comprehensive patient health information, including symptoms, medical history, examination results, imaging reports, laboratory tests, diagnoses, and treatment plans.
  • Ensuring that all relevant reports, lab results, and patient demographics are forwarded to external facilities prior to appointments.
  • Reviewing forms to confirm completion, proper identification, signatures, and necessary information in the Electronic Health Record (EHR).
  • Communicating with physicians and healthcare professionals to clarify diagnoses or gather additional information as needed.
  • Collecting lab and imaging reports, hospital notes, and referral details.
  • Verifying insurance coverage and patient demographic information.
  • Updating patient charts to ensure completeness and proper filing.
  • Facilitating timely delivery of external communications to the physician.

Qualifications:

Education:

  • High school diploma; some college education preferred.

Experience:

  • At least one year of experience in a medical office or healthcare facility.
  • Experience with Electronic Health Records (EHR) and scheduling systems.
  • Proven experience in customer service, both in-person and over the phone.

Physical Requirements:

  • This role requires regular walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
  • Ability to lift and support weights up to 35 pounds.
  • Strong attention to detail is essential.
  • Prolonged use of a computer is required.
