Patient Care Coordinator - Overview:

The Patient Care Coordinator plays a crucial role in ensuring that the physician's treatment plan is executed accurately and promptly.

Key responsibilities include managing and facilitating referrals with healthcare providers, hospitals, and community resources, scheduling follow-up appointments for patients, and addressing patient inquiries regarding access to healthcare services.



Core Responsibilities:

Patient Coordination Duties:
• Timely acquisition of ancillary results for all physician orders.
• Securing clinic notes from referring providers for all referred patients promptly.
• Responsible for documenting and obtaining authorization from insurance companies for scheduled referrals and procedures.
• Flexibility to work at various Tryon office locations as required.
• Regularly monitor assigned Athena boxes and other designated communication channels.
• Additional responsibilities as assigned.

Clinic Support Duties:


• Compiling comprehensive health information for patients, including symptoms, medical history, examination results, imaging reports, laboratory tests, diagnoses, and treatment plans.

• Ensuring that all pertinent reports, lab results, and demographic information are forwarded to external facilities prior to patient appointments.

• Reviewing documentation to confirm that all forms are completed, accurately identified, and signed, with all necessary information entered into the Electronic Health Record (EHR).

• Engaging with physicians and other healthcare professionals as needed to clarify diagnoses or gather additional information.

• Collecting lab and imaging reports, hospital notes, and referral details; verifying insurance coverage and patient demographics; updating charts to maintain completeness and proper filing.

• Ensuring the timely delivery of external communications to the physician.
Qualifications:
Education:
• High school diploma required; some college coursework preferred.
Experience:
• At least one year of experience in a medical office or healthcare setting.
• Proficiency in Electronic Health Records (EHR) and scheduling systems.
• Experience in customer service, both in-person and via phone.
Physical Requirements:
• Role involves consistent walking, standing, sitting, lifting, reaching, stooping, bending, pushing, and pulling.
• Ability to lift and support weights up to 35 pounds.
• Strong attention to detail is essential.
• Prolonged use of a computer is required.
