Job SummaryWe are seeking a highly motivated and compassionate Dialysis Coordinator II to join our team. As a Dialysis Coordinator, you will play a critical role in ensuring the smooth operation of our dialysis services, working closely with patients, families, and healthcare teams to provide personalized care and support.
Key Responsibilities- Coordinate patient care and services, including scheduling, transportation, and medication management
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions
- Collaborate with multidisciplinary teams to develop and implement individualized care plans
- Provide education and support to patients and families on dialysis treatment options, medication management, and lifestyle changes
- Participate in quality improvement initiatives to enhance patient outcomes and satisfaction
- High school diploma or equivalent required; bachelor's degree preferred
- 1-2 years of experience in healthcare or a related field
- Strong communication and interpersonal skills
- Ability to work in a fast-paced environment and prioritize multiple tasks
- Basic computer skills and proficiency in electronic medical records
