DaVita Clinical Coordinator Job Description

We are seeking a skilled Clinical Coordinator to join our team at DaVita, a leading provider of dialysis services. As a Clinical Coordinator, you will play a critical role in ensuring the highest level of care for our patients with end-stage renal disease (ESRD) or chronic kidney disease.

Key Responsibilities:
  • Coordinate patient care and services, including trend lab values, vital signs, and outcome management.
  • Ensure the safety, comfort, and well-being of patients by delivering high-quality care and services.
  • Collaborate with the clinical team to develop and implement individualized care plans.
  • Provide training and education to patients and their families on dialysis treatment and self-care.
  • Participate in quality improvement initiatives to enhance patient outcomes and satisfaction.
Requirements:
  • Current Registered Nurse (RN) license in the state of practice.
  • At least 18 months of registered nursing experience.
  • Dialysis Charge RN readiness approval by Clinical Nurse Manager or RN-licensed FA/GFA required.
  • Certified Nephrology Nurse (CNN) or Certified Dialysis Nurse (CDN) preferred.
