Patient Care Manager

We are seeking a highly skilled and compassionate Patient Care Manager to join our team at LHC Group. As a Patient Care Manager, you will be responsible for supervising and coordinating clinical services, providing quality support to patient-facing teams, and ensuring that patients receive the highest level of care.
Key Responsibilities- Supervise and coordinate clinical services to ensure high-quality patient care
- Provide leadership and guidance to patient-facing teams to ensure effective communication and collaboration
- Ensure that patient needs are continually assessed and care rendered is individualized to patient needs
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals
- Review assessments and plans of care daily to ensure that care is appropriate and reasonable
- Current RN licensure in state of practice
- Current CPR certification
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
