LPN - Home Health - PRN: Delivering Compassionate Care in the Comfort of Home

We are seeking a skilled and compassionate LPN to join our team at Enhabit Home Health & Hospice. As a LPN, you will play a vital role in delivering high-quality nursing care to patients in the comfort of their own homes.
Key Responsibilities- Visit patients in their homes and provide nursing care under the direction of RNs and in compliance with physicians' orders.
- Develop and implement personalized care plans to meet the unique needs of each patient.
- Collaborate with the healthcare team to ensure seamless care coordination and communication.
- Provide education and support to patients and their families to promote healthy lifestyle choices and disease management.
- Current licensure as a Licensed Practical Nurse in the state of employment.
- Valid state driver's license and automobile liability insurance.
- Dependable transportation and ability to drive in all types of weather conditions.
- Ability to work independently and as part of a team to achieve patient-centered care.
