Youth Support Partner - Children's Intensive Services

Position: Youth Support Partner – Full Time (40 hours/week)
Location: Compass Health

Compass Health is actively seeking a Youth Support Partner to be part of the Children's Intensive Services program.

The Children's Intensive Outpatient Services program is designed to assist children and their families within their community, utilizing a least restrictive approach through the Wraparound process. This method emphasizes family voice and choice, identifies strengths, builds natural supports, and encourages self-sufficiency. Wraparound teams deliver comprehensive behavioral health services and support to Medicaid-eligible individuals, up to 21 years old, who have complex behavioral health needs, along with their families. The primary objective of this program is to enable eligible youth to thrive in their homes and communities while minimizing or preventing disruptive out-of-home placements. Assessment, treatment, and support services are delivered in the youth and family's natural environments, such as home, school, and community. Families have reported positive outcomes, including enhanced stability, increased support, community connections, and renewed hope for the future.

Key Responsibilities

The ideal candidate will collaborate as part of a multi-disciplinary treatment team to provide a tailored, innovative, and strength-based approach within the Wraparound model. This role focuses on supporting youth and caregivers to maintain children in a community-based environment.

This position requires a flexible schedule, including evening and weekend hours, both in the office and within the community. Youth Support Partners will work closely with clients to encourage their engagement and active participation in the decision-making process that drives the Wraparound approach. They ensure that each client's voice is heard and that their unique needs are addressed.
