Your expertise will contribute to the mission of The Columbus Organization, a leading provider in Delaware's Targeted Case Management Services.
Key Responsibilities:- Conduct interviews and assess personal, health, employment, social, or financial needs in alignment with program standards.
- Engage in case management activities, including assessing, planning, developing, implementing, monitoring, and evaluating services tailored to individual human service needs.
- Make informed recommendations as part of client service plans, which may involve clinical treatment, counseling, or eligibility determinations for health or human services.
- Interpret relevant laws, regulations, and policies to ensure compliance and effective service delivery.
- Utilize automated information systems for data management, including entering, updating, and reporting on client information.
- Prepare comprehensive narrative reports to document client progress and service outcomes.
- Maintain a valid state driver's license and automobile insurance, ensuring reliable transportation for client visits.
Qualifications:- A minimum of one year of experience in health or human services support.
- At least six months of experience in case management.
- Proficiency in report writing and data management.
- Associates Degree is preferred but not mandatory.
- Bilingual (Spanish) skills are a plus.
