POSITION SUMMARY:


The Care Coordination Supervisor is responsible for overseeing and directing all activities of the Care Management team. This role involves providing guidance, support, and mentorship to both Care Management and Social Work staff.

Key responsibilities include ensuring effective Discharge Planning and Transitions of Care within the hospital setting. The supervisor will monitor quality metrics to guarantee that appropriate social and transitional services are delivered to patients and their families.

Additionally, the role requires the development and maintenance of collaborative relationships with physicians, nursing leadership, insurance providers, and community organizations to address the needs of vulnerable populations, including those who are uninsured or underinsured.


EDUCATIONAL REQUIREMENTS:


A minimum of an Associate's Degree in Nursing from an accredited institution or a Master's Degree in Social Work, along with a valid Social Work license in the State of Georgia.


EXPERIENCE REQUIREMENTS:


At least three (3) years of relevant experience in care management, medical social work, or transitional care management is required.


LICENSURE/CERTIFICATION REQUIREMENTS:


Must possess a current license as a Registered Nurse in Georgia or a multistate license, or be a Licensed Master Social Worker (LMSW) in Georgia.


PREFERRED QUALIFICATIONS:


A Bachelor's Degree in Nursing from an accredited institution is preferred.
