The Care Coordinator plays a pivotal role in managing the care of high-risk and complex patients. This position is essential in bridging care gaps and assisting patients during transitions in their healthcare journey. The Care Coordinator is dedicated to enhancing patient health outcomes by providing guidance and advocacy, focusing on reducing emergency room visits, minimizing hospital readmissions, and managing chronic health conditions effectively.

Key Responsibilities

Patient Care and Support:

  • Evaluates the comprehensive needs of patients, including physical, social, and environmental factors.
  • Educates patients and their families about diagnoses, treatment options, and self-care strategies.
  • Facilitates continuity of care by coordinating with other healthcare providers and community resources.
  • Empowers patients in self-management and shared decision-making processes.
  • Conducts medication reconciliation in complex scenarios as needed.
  • Advocates for patient rights and ensures their dignity and autonomy are respected.
  • Participates in interdisciplinary team meetings to enhance patient care management.
  • Conducts community visits to identify healthcare barriers and provide necessary resources.
  • Collaborates with various clinic disciplines to ensure optimal patient care.

Documentation and Compliance:

  • Maintains accurate and timely documentation of all patient interactions in the electronic health record (EHR).
  • Ensures adherence to practice policies and compliance with regulatory standards.
  • Tracks and reports on care coordination activities for internal and external stakeholders.
  • Completes additional tasks as assigned by the Clinical Operations Manager.

Qualifications

Education and Experience:

  • Bachelor's degree in Nursing.
  • Experience in chronic disease management and patient education is preferred.
  • Familiarity with population health management principles.
  • Valid and unrestricted Registered Nurse license.
