Job Overview

POSITION SUMMARY
Under the guidance of the Non-Residential Supervisor, this role is dedicated to delivering comprehensive and holistic support services to clients and their families.


KEY RESPONSIBILITIES include, but are not limited to:

  • Facilitating services aimed at enhancing family independence.
  • Assisting clients in formulating safety plans, setting objectives, and identifying resources to meet their needs.
  • Collaborating with relevant staff to address client requirements.
  • Responding to urgent calls and providing necessary information and referrals.
  • Conducting client intakes, evaluations, and exits in accordance with established policies.
  • Maintaining daily documentation and client records as required.
  • Offering information, referrals, crisis intervention, and advocacy for program participants.
  • Documenting and reporting to relevant health and family services.
  • Participating in case meetings, training sessions, and staff gatherings as assigned.
  • Engaging in public speaking and representing the agency at community events.
  • Participating in on-call duties as necessary.
  • Providing agency services to various organizations, including hospitals and law enforcement.
  • Maintaining an updated directory of community resources.
  • Facilitating client transitions and follow-up services.
  • Managing phone communications when the office is unattended.
  • Performing additional tasks as required.

QUALIFICATIONS
To be successful in this role, candidates must demonstrate the ability to perform essential duties effectively. The following qualifications are essential:

  • Bachelor's Degree in Social Work or a related field, or equivalent experience.
  • A minimum of two years of experience in a human services environment or comparable experience.
  • Proficient in computer data entry.

LANGUAGE PROFICIENCY
Ability to read, analyze, and interpret various documents, including business reports and legal materials. Proficient in writing reports, correspondence, and manuals that adhere to established formats. Capable of effectively presenting information and addressing inquiries from various stakeholders.


MATHEMATICAL ABILITY
Ability to apply basic mathematical concepts.


PROBLEM-SOLVING SKILLS
Ability to address practical issues and navigate diverse situations with limited standardization.


CERTIFICATIONS
Must meet certification standards as required and possess or be able to obtain a valid driver's license.


PHYSICAL REQUIREMENTS
The physical demands of this position require regular sitting, use of hands, and effective communication. Occasional lifting of up to 20 pounds may be necessary.


WORK ENVIRONMENT
The work environment is typically moderate in noise level, with necessary accommodations for individuals with disabilities.
