Position Overview

SUMMARY
Under the guidance of the Non-Residential Supervisor, this role is dedicated to delivering comprehensive and holistic support services to clients and their families.


KEY RESPONSIBILITIES include, but are not limited to:

  • Facilitating services aimed at enhancing family self-sufficiency.
  • Assisting clients in developing safety plans, setting objectives, and identifying resources for achievement.
  • Coordinating client needs with relevant personnel.
  • Responding to crisis situations; providing necessary information and referrals.
  • Conducting client intakes, assessments, and exits in accordance with established policies and procedures.
  • Maintaining daily contact logs and other required documentation.
  • Managing client files as per organizational guidelines.
  • Offering information, referrals, crisis intervention, and advocacy for program participants.
  • Documenting and reporting to relevant health and family services.
  • Participating in case meetings, training sessions, and staff gatherings as required.
  • Engaging in public speaking and representing the organization in community activities.
  • Being part of an on-call rotation as necessary.
  • Providing services to various community agencies, including hospitals and law enforcement.
  • Maintaining a directory of local resources.
  • Facilitating client admissions and follow-up services post-exit.
  • Managing phone lines during office hours.
  • Performing additional duties as assigned.

QUALIFICATIONS
To be successful in this role, candidates must meet the following criteria:

  • Bachelor's Degree in Social Work or a related field, or equivalent experience.
  • A minimum of two years of experience in a human services environment or equivalent.
  • Proficiency in computer data entry.

LANGUAGE PROFICIENCY
Ability to read, analyze, and interpret various business documents, including reports and regulations. Capable of writing clear reports and correspondence that adhere to established formats. Effective communication skills for presenting information and addressing inquiries from various stakeholders.


MATHEMATICAL APTITUDE
Ability to apply basic mathematical concepts.


PROBLEM-SOLVING SKILLS
Ability to tackle practical issues and manage diverse variables in situations with limited standardization.


CERTIFICATIONS
Must comply with certification standards as directed by supervisory personnel. A valid driver's license is required.


PHYSICAL REQUIREMENTS
The physical demands for this position include regular sitting, use of hands for various tasks, and effective communication. Occasional lifting of up to 20 pounds may be required.


WORK ENVIRONMENT
The work environment is typically moderate in noise level and includes the use of standard office equipment.
