SUMMARY
Under the guidance of the Non-Residential Supervisor, this role offers comprehensive, holistic support to clients and their families within the community.
KEY RESPONSIBILITIES include, but are not limited to:
- Facilitating services aimed at enhancing family self-sufficiency.
- Assisting clients in developing safety plans, setting objectives, and identifying resources to meet their needs.
- Coordinating client requirements with relevant personnel.
- Responding to crisis situations, providing necessary information and referrals.
- Conducting client intakes, assessments, and exits in accordance with established policies.
- Maintaining accurate documentation and daily contact records as required.
- Managing client files in compliance with organizational policies.
- Offering information, referrals, crisis intervention, and advocacy services to program participants.
- Documenting and reporting as necessary to relevant health and family services.
- Participating in case meetings, training sessions, and staff gatherings as assigned.
- Engaging in public speaking engagements and representing the organization in community initiatives.
- Participating in on-call duties as needed.
- Providing services to hospitals, social service organizations, educational institutions, and law enforcement.
- Maintaining a directory of community resources.
- Facilitating client admissions and follow-up services post-exit.
- Managing phone communications when the office is unattended.
- Performing additional tasks as assigned.
QUALIFICATIONS
To excel in this position, candidates must meet the following criteria:
- Bachelor's Degree in Social Work or a related discipline, or equivalent experience.
- A minimum of two years of experience in a human services environment or comparable background.
- Proficient in computer data entry.
LANGUAGE PROFICIENCY
Ability to read, analyze, and interpret various documents, including business publications, financial reports, and legal documents. Proficient in writing reports, correspondence, and procedural manuals that adhere to established formats. Capable of effectively presenting information and addressing inquiries from supervisors, clients, colleagues, and the public.
MATHEMATICAL ABILITY
Ability to apply basic mathematical concepts and principles.
REASONING SKILLS
Ability to tackle practical problems and manage a variety of concrete variables in situations with limited standardization. Ability to interpret diverse instructions in written, oral, diagrammatic, or schedule formats.
CERTIFICATIONS
Must fulfill certification requirements as directed by the supervisor. Must possess or be eligible to obtain a valid driver's license.
