Empowering Individuals with Peer Support

MetroPlusHealth, a leading healthcare provider in New York City, is seeking a highly motivated Peer Specialist Advocate to join our team.

We are committed to delivering high-quality care and support to our members, particularly those with Serious Mental Illness (SMI) and Substance Abuse Disorder History. As a Peer Specialist Advocate, you will play a vital role in providing supportive services, promoting recovery, and empowering individuals to achieve their goals.

About the Role:

  • Provide one-to-one sessions to assist clients in articulating personal goals for recovery and developing recovery plans.
  • Work collaboratively with clients' primary MetroPlusHealth Behavioral Health Care Manager and community treatment teams to determine steps for achieving goals and self-directed recovery.
  • Complete Housing Applications and facilitate Supportive Housing placements, escorting members as needed.
  • Arrange transportation for scheduled PCP Medical Appointments.
  • Teach Recovery Dialogues and help members overcome relapse fears.
  • Support members working with assigned Parole Officers.
  • Participate in BH HARP Team meetings, supervision, discharge planning, and clinical rounds discussions.
  • Accompany HARP members to initial HCBS appointments if their CMA worker is unavailable.
  • Utilize personal lived experience to empower individuals to define and pursue their own strength-based recovery and wellness goals.
  • Assist clients in building social skills that enhance their quality of life.
  • Provide peer support during crisis situations.

Requirements:

  • High School graduation or equivalent; 5 years of professional Behavioral Health experience, or an equivalent combination of training, education, and experience.
  • Knowledge of SMI and/or SUD concerns and challenges; Recovery process; and ability to facilitate recovery using standardized mental health processes.
  • Computer literacy with Microsoft Office (Word, Outlook, Teams); working knowledge of electronic medical record systems.
  • Bilingual English/Spanish preferred; fluency in other languages welcomed.

Salary Information:

The estimated annual salary for this position is $51,500.00, commensurate with experience and qualifications.
