Retention and Adherence Specialist

The Retention and Adherence Specialist plays a critical role in supporting patients living with HIV who have not achieved viral suppression. This position conducts outreach, intake, assessments, screenings, and medical case management to ensure patients receive the necessary care and support.
Key Responsibilities- Conduct outreach and engagement activities to link patients to care and re-engage patients who have fallen out of care.
- Manage a caseload of patients and complete initial assessments, reassessments, case conference notes, and service plans to determine progress in the program.
- Assist patients with no insurance in obtaining ADAP, Medicaid, or other insurances.
- Conduct eligibility screenings and intakes using approved protocols and forms, and document information in the electronic medical record.
- Present cases at multidisciplinary case conferences to discuss patient progress and barriers to achieving viral suppression, and document outcomes in the EMR.
- Collaborate with patients and peer educators to develop plans to meet patient care needs.
- Conduct home visits to re-engage patients lost to care.
- Document all patient encounters and attempts to contact patients in progress notes in the EMR.
- Complete daily AIRS encounter forms and log sheets, and submit to the Data Management Specialist for entry into AIRS.
- Assist or lead adherence group activities with the Program Manager and Peer Navigator.
- Attend HIV/AIDS forums, work groups, and conferences to stay up-to-date on trends and changes in HIV-related care and treatment.
- Provide timely and appropriate referrals to onsite and offsite service providers as needed.
- Participate in QA/PI activities.
- High school or GED required; Bachelor's degree in Human Services or related field preferred.
- One to five years of experience working in HIV prevention, treatment, and adherence; minimum of three years of job-related experience.
- Ability to use verbal and nonverbal calming techniques to prevent crisis situations.
- Proficiency with Office PCs, desktops, and laptop software, including Microsoft Office.
- Ability to communicate effectively in English, both verbally and in writing.
- Good interpersonal skills and ability to work effectively in a team environment.
