Position Summary: The Comprehensive Partnership Case Manager plays a vital role in delivering individualized and group rehabilitative services aimed at enhancing, sustaining, and/or restoring client functionality.
Key Responsibilities:
- Facilitate access to community resources, including housing, healthcare, and financial assistance for clients.
- Accompany clients in community settings to improve their access to necessary resources and support during appointments.
- Participate in court proceedings, prepare progress documentation, and collaborate closely with judicial and probation personnel.
- Engage in a multidisciplinary treatment team comprising Care Coordinators, Peer Support Specialists, Clinicians, and Therapists to optimize clinical outcomes for clients.
- Ensure the maintenance of accurate, complete, and organized clinical records, with a preference for familiarity with relevant mental health documentation.
- Provide crisis intervention as required.
- Perform additional duties as assigned.
Essential Skills:
- Proven experience and knowledge in the mental health sector.
- Empathy for clients and a commitment to making a positive impact.
- Ability to work effectively with a diverse clientele, including those with severe and persistent mental health conditions.
- Excellent verbal and written communication abilities.
- Comfortable engaging with clients from the criminal justice system.
- Capacity to work autonomously in a dynamic environment, both in office settings and the community.
- Willingness to undertake after-hours on-call responsibilities.
- Ability to uphold productivity and documentation standards.
- Strong time management skills, with the ability to prioritize tasks and demonstrate sound judgment.
- Detail-oriented with the capability to manage multiple tasks and demonstrate initiative.
- Familiarity with community resources related to mental health and substance abuse, alongside experience in a multidisciplinary team setting.
- Ability to engage and support colleagues and clients from various cultural, ethnic, and economic backgrounds.
- Proficient in standard office software, including word processing, spreadsheets, and data collection programs.
Work Environment and Physical Requirements:
This role involves active engagement in field, residential, and/or office environments. Employees must be prepared to interact with individuals in crisis, who may present various challenges, including health and hygiene issues. The position requires:
- High levels of concentration and attention to detail over extended periods.
- Effective communication skills across all levels of the organization and with external partners.
- Ability to organize thoughts quickly and understand communications from others.
- Physical capabilities to stand, sit, walk, and perform tasks requiring dexterity.
- Occasional lifting or carrying of items up to 25 lbs.
