As a Social Worker MSW, you will play a vital role in providing psychosocial support to patients and their families, utilizing your expertise to identify needs, goals, and interventions that align with our established plan of care.

Responsibilities
  • Conduct comprehensive psychosocial assessments to identify patient and family needs
  • Develop and implement individualized plans of care that address psychosocial needs
  • Collaborate with the interdisciplinary team to ensure comprehensive care
  • Provide emotional support and counseling to patients and families
  • Document patient progress and outcomes
Requirements
  • Master's Degree in Social Work from a CSWE-accredited school
  • Licensure as required by the state in which the hospice is located
  • One year of experience as a Social Worker in a healthcare and/or hospice setting, with three to five years preferred
