Compassionate Social Worker (Hospice) Opportunity

At Medical Services of America, we are seeking a dedicated and compassionate Full-Time MSW to join our Hospice team in Columbia, Richmond and McDuffie Counties. As a vital member of our interdisciplinary team, you will provide high-quality social work services to patients and families, ensuring they receive the support and care they need during this challenging time.



The ideal candidate will possess a Master's or Bachelor's level social work degree from a school of social work accredited or approved by the Council of Social Work Education, and be currently licensed to practice as a social worker in the state of GA. A minimum of one-year Hospice experience is preferred, but not required. We offer a competitive salary of $65,000 - $85,000 per year, depending on experience, plus excellent benefits, including generous paid time off, medical/prescription, dental & vision insurance, company-paid employee life insurance, 401(k) retirement with a generous company match, and many other great benefits.



About Our Hospice Program

Our Hospice program is designed to provide comprehensive care and support to patients and families, focusing on their physical, emotional, and spiritual needs. As a Hospice social worker, you will work closely with our interdisciplinary team to assess patients' needs, develop individualized care plans, and provide bereavement counseling and support to patients and families.



Responsibilities


  • Provide Social Work Services: Work with patients and families to assess their needs, develop care plans, and provide bereavement counseling and support.
  • Collaborate with Interdisciplinary Team: Work closely with our interdisciplinary team to develop and implement individualized care plans.
  • Communicate Effectively: Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Maintain Accurate Records: Maintain accurate and up-to-date records of patient interactions, care plans, and other relevant information.
