Job Overview
In this role as a Patient Care Specialist, you will play a vital part in the delivery of high-quality patient care at Bluestem Health. With a strong focus on chronic disease management, you will work closely with healthcare providers to develop and implement comprehensive care plans that cater to the unique needs of each patient.

About You
We are seeking a highly skilled and compassionate Registered Nurse to join our team. As a Chronic Disease Program Manager, you will be responsible for leading the development and implementation of protocols and standards of care for patients with chronic diseases. Your exceptional communication skills and ability to build strong relationships with patients, families, and healthcare teams will be invaluable in this role.

Key Responsibilities
- Promote teamwork and enhance a positive work environment by fostering a culture of respect and open communication among colleagues
- Develop and maintain library of health promotion and patient education materials and resources to support patients' understanding of their conditions and treatment options
- Conduct thorough assessments of patients' charts to identify gaps in care and develop personalized care plans
- Collaborate with consulting providers, nursing staff, and behavioral health consultants to ensure seamless coordination of patient care

Requirements
- RN license in the State of Nebraska; BSN preferred
- 3 years of nursing experience, preferably in a related field
- Excellent problem-solving skills and ability to work independently with minimal supervision
- Strong computer skills and Electronic Health Record experience a plus
