Social Services Specialist

St. Charles Health System is seeking a highly skilled Social Services Specialist to join our team. As a key member of our interdisciplinary care team, you will play a vital role in ensuring the psychosocial needs of our patients and their families are met.
Job Summary:The Social Services Specialist will assist in meeting the psychosocial needs of patients and their families by helping them utilize services to achieve the optimum level of function throughout the continuum of care. This includes providing supportive counseling, end-of-life support, care planning, and brief interventions for patients experiencing coexisting medical concerns and social/behavioral/mental health issues.
Key Responsibilities:- Function as a member of the Interdisciplinary Care Team to assure care is accomplished effectively and efficiently in a cost-effective manner.
- Coordinate care plans as a consultative service with collaboration of interdisciplinary teams to address psychosocial needs related to injuries, accidents, and critical incidents.
- Advocate for patient's needs, ensuring they receive appropriate services and resources to assist in continuum of care and discharge planning.
- Provide bio-psychosocial and trauma-informed interventions, support, and referrals to patients and families.
- Collaborate to identify coping strategies and support mechanisms.
- Identify and communicate community resources, acting as a resource for patients and families throughout the continuum of care.
- Provide brief, evidence-based interventions for emotional distress.
- Assess threats of self-harm or harm to others.
- Provide education, support, community resources, and care planning for palliative care, hospice, and bereavement needs.
- Conduct comprehensive assessments of economic, mental health, family dynamics, and social support systems.
- Evaluate Social Determinants of Health and connect patients with appropriate resources.
- Provide substance abuse assessments and brief interventions (SBIRT, DAST) and refer to outpatient behavioral health for follow-up for patients scoring positively on the Columbia Suicide Risk Assessment.
- Identify victims of child abuse, domestic violence, and elder abuse. Make appropriate referrals and notify the Department of Human Services (DHS) and/or law enforcement when applicable.
- Provide information, education, and resource referrals related to advance directives, power of attorneys, guardianship, conservatorship, and long-term care services.
- Assist in navigating legal avenues for guardianship and navigating pathways through local and state resources.
- Master's level degree in Social Work or Behavioral Sciences.
- Valid current Oregon driver's license and ability to meet SCHS driving requirements for Home Health, Hospice, and Behavioral Health departments.
- One (1) year experience in a healthcare or social work-related field.
- Preferred: Two (2) years of experience in a hospital setting.
