Job Title: Intake Referral Specialist

Job Summary: We are seeking a highly skilled Intake Referral Specialist to join our team at St. Charles Health System. As a key member of our Behavioral Health department, you will play a vital role in ensuring that patients receive the necessary support and services to achieve optimal levels of functioning.

Key Responsibilities:

  • Assess patients' needs and accurately document information in a timely manner, demonstrating compliance with department standards.
  • Perform patient screening/assessment of social, economic, emotional, and family situations.
  • Coordinate with clinicians and preauthorization staff to ensure timely start of services for new clients.
  • Establish and maintain relationships interdepartmentally and within assigned departments to ensure customer and caregiver satisfaction with referrals/admissions/appointments.
  • Maintain a thorough knowledge of community resources.
  • Process orders/referrals from IBH providers from SCMG practices.
  • Provide immediate response to intakes with contracted OPBH specialty medical groups.
  • Stay informed regarding SCHS Disaster Plan and act as first responder to Disasters for OPBH.
  • Work with Sage View staff to coordinate aftercare services for Sage View clients.
  • Coordinate with Emergency Department Social Workers and the Mobile Crises Assessment Team (MCAT) regarding patient mental health needs.
  • Oversee OPBH claims denial management and coordinate efforts with the CBO.
  • Provide consultation to clinic providers regarding clinical, ethical, privacy practices and CPT and DSM coding changes and protocols.
  • Oversee daily operation of the Caregiver Assistance (CAP) and EAP Contracted Provider Programs.

Requirements:

  • Bachelor's Degree in Social Work, Behavioral Science, or other related field.
  • Three (3) years Mental Health related experience.
  • Valid Oregon driver's license and ability to meet St. Charles Health System driving requirements.

Preferred Qualifications:

  • Master's Degree in Social Work, Behavioral Science, or other related field.
  • Registered Bachelor's Social Worker preferred (RBSW).
  • Clinical Licensure with State of Oregon.
  • Member of the National Association of Social Worker's or other national professional association membership.

Working Conditions:

  • Must be able to wear appropriate Personal Protective Equipment (PPE) required to perform the job safely.

Physical Requirements:

  • Continually (75% or more): Use of clear and audible speaking voice and the ability to hear normal speech level.
  • Frequently (50%): Sitting, keyboard operation.
  • Occasionally (25%): Standing, walking, bending, stooping/kneeling/crouching, climbing stairs, reaching overhead, lifting/carrying/pushing or pulling 1-10 pounds, grasping/squeezing, operation of a motor vehicle, ability to hear whispered speech level.
  • Never (0%): Climbing ladder/step-stool, lifting/carrying/pushing or pulling 11-50 pounds.

Exposure to Elemental Factors:

  • Occasionally (25%): Chemical solution for Laboratory Caregivers.
  • Rarely (10%): Wet/slippery area.
  • Never (0%): Heat, cold, noise, dust, vibration, chemical solution, uneven surface.
