Job Summary

Partners In Care is seeking a skilled Palliative Care Social Worker to join our team in Central Oregon. As a Palliative Care Social Worker, you will play a vital role in providing compassionate care to patients and their families during challenging times.

Key Responsibilities
  • Conduct initial and ongoing psychosocial and emotional assessments of patients and families.
  • Utilize therapeutic techniques to support patients and families in understanding their options and participating in decision-making around their health needs.
  • Provide emotional support counseling and complex care management services to palliative care patients and families.
  • Facilitate care coordination and make appropriate referrals based on identified needs.
  • Perform advance care planning, including end-of-life conversations, clarifying medical care goals, and discussing hospice care and bills appropriately.
  • Assist with patient and family education, including health literacy, cognitive abilities, communication patterns, challenges around their medical care, and learning styles.
  • Engage patients, hospitals, facilities, and the community in education about palliative care and services provided.
  • Work collaboratively with palliative care providers to ensure appropriate follow-up for patient needs.
  • Work collaboratively with community resources to support patients and help ensure continuity of care across service lines and settings, advocating for patient-centered care.
  • Assess and support patients and families regarding anticipatory grief and explores family systems/dynamics and coping methods. Evaluate the need for bereavement counseling or appropriate interventions related to complicated grief.
  • Facilitate appropriate and measurable goals to help patients achieve their desired quality of life.
Requirements
  • Graduate of a Masters program in Social Work accredited by the Council on Social Work Education.
  • 1-3 years of applicable experience in end-of-life care in a healthcare setting.
  • Valid State of Oregon Clinical Social Worker license (LCSW) for social work applicants.
  • Specialized certification in palliative care social work is preferred.
  • Ability to pass a background test, drug screen, and TB test.
  • Valid CPR Certification.
  • For those working in the field, current automobile insurance, a valid Oregon drivers license, and reliable transportation are required.
  • Knowledge of Federal, State, and Local regulations, Medicare Conditions of Participation, and hospice Medicare benefits.
  • Knowledge of HIPAA privacy rules and regulations.
  • Knowledge of electronic medical records systems.
  • Demonstrate excellent organizational skills and ability to prioritize and meet deadlines.
  • Strong communication and interpersonal skills.
  • Ability to work independently and collaboratively within an interdisciplinary team.
  • Compassion, empathy, and dedication to patient care.
