About the Role

We are seeking a highly skilled and experienced Care Coordination Supervisor to join our team. As a key member of our leadership team, you will be responsible for overseeing a team of Care Coordinators and ensuring the effective delivery of care coordination services.

Key Responsibilities
  • Oversee a team of Care Coordinators and provide guidance and support as needed
  • Develop and implement care coordination strategies to improve outcomes and efficiency
  • Manage the overall case load for your region and ensure timely and effective case resolution
  • Support and develop your team members to ensure they have the skills and knowledge needed to succeed in their roles
  • Collaborate with other departments and stakeholders to ensure seamless communication and coordination
Requirements
  • Bachelor's degree in a related field
  • At least 2 years of experience as a Case Manager or Care Coordinator
  • At least 1 year of experience in a Supervisory role
  • At least 1 year of experience supporting IDD populations
  • Medicare knowledge
