Position Overview

Role Summary:

The Scheduler/Client Service Coordinator plays a vital role in ensuring seamless patient care through effective scheduling and communication.

Key Responsibilities:

  • Manage and input data for new and existing patient and employee records into the system.
  • Coordinate patient appointments and track supervisory visits in compliance with regulatory standards.
  • Communicate with clinical supervisors and case managers when verbal orders are required for care initiation.
  • Ensure satisfaction for both patients and clinicians by maintaining consistent scheduling.
  • Handle service requests and scheduling modifications while managing patient and resource calendars.
  • Report staffing needs and performance issues to the Clinical Supervisor as necessary.
  • Notify relevant departments regarding additional payer authorizations for ongoing services.
  • Document interactions with patients, families, or caregivers in the Electronic Medical Record.
  • Provide support to other team members as needed.
  • Engage in information management and performance improvement initiatives.
  • Participate in training and educational programs to enhance skills and knowledge.
  • Stay informed about new methods and trends in the industry and share insights with colleagues.
  • Perform additional administrative tasks as assigned.
  • Input scheduling data and maintain staff schedules, ensuring appointments align with care plans and availability.
  • Communicate with care providers and clients regarding daily changes.
  • Verify caregiver qualifications and ensure all documentation is complete.
  • Review client information and care plans upon intake to ensure proper monitoring and staffing.
  • Assign appropriately skilled personnel to new clients and update scheduling information promptly.

Qualifications:

Must hold an LPN or RN degree with a minimum of two years of customer service experience. Previous healthcare or home care experience is preferred, along with at least one year of scheduling experience.

Skills Required:

  • Exceptional customer service and communication skills.
  • Strong computer literacy, particularly in Microsoft Office.
  • Ability to work independently and as part of a team.
  • Sound judgment and decision-making capabilities.
  • Commitment to maintaining confidentiality and handling sensitive information.
