Job Overview

Position Summary:

The Client Service Scheduler/Coordinator plays a vital role in ensuring seamless patient care and satisfaction. This position requires a strong focus on customer service and effective communication.

Key Responsibilities:

  • Manage and input data for both new and existing patient and employee records into the system.
  • Coordinate patient appointments and monitor compliance with regulatory visit requirements.
  • Communicate with Clinical Supervisors and case managers regarding necessary verbal orders for care initiation.
  • Ensure satisfaction for both patients and clinicians by maintaining consistent scheduling.
  • Handle service requests and scheduling modifications while keeping patient and resource calendars updated.
  • Report staffing needs and performance issues to the Clinical Supervisor as necessary.
  • Notify relevant departments about additional payer authorizations for ongoing services.
  • Document interactions with patients, families, or caregivers in the Electronic Medical Record.
  • Provide support to other team members as required.
  • Engage in information management and performance improvement initiatives.
  • Participate in training and educational programs to enhance skills and knowledge.
  • Stay informed about industry trends and communicate relevant updates to the team.
  • Perform additional administrative tasks as assigned.
  • Input scheduling data and maintain staff schedules, ensuring appointments align with care plans and availability.
  • Contact care providers and clients regarding daily scheduling changes.
  • Verify caregiver licensing and certifications to ensure compliance.
  • Review documentation to confirm all forms are completed accurately.
  • Ensure all client files are comprehensive and contain required documents.
  • Assess client information and care plans upon intake to facilitate monitoring and staffing progress.
  • Assign appropriately skilled caregivers to new clients and promptly update scheduling information in the database.

Qualifications:

Must hold an LPN or RN degree.

Experience:

Minimum of 2 years in customer service is required, with preference given to candidates with a year of healthcare or home care experience and scheduling expertise.
