Position Overview

Objective of the Role

The main objective of this role is to deliver rehabilitation services to residents, aiding them in enhancing mobility across various levels of care, thereby improving their functional capabilities and overall quality of life. The Physical Therapist will collaborate closely with the interdisciplinary team to assess and treat residents effectively.

Authority Delegation

As a Physical Therapist, you are entrusted with the administrative authority, responsibility, and accountability necessary to fulfill your designated duties.

Key Responsibilities

This job description aims to encompass the essential functions of your role; however, it does not limit the scope of duties you may be required to perform. The absence of specific duties does not exclude them from your responsibilities if they are similar or related.

Primary Duties & Responsibilities

  • Assess residents to identify their physical therapy requirements, including their strengths, limitations, and potential for improvement.
  • Design and execute tailored treatment plans aimed at enhancing residents' strength, flexibility, and range of motion.
  • Adjust treatment methodologies as necessary to accommodate the unique needs of each resident.
  • Document residents' progress and share findings with the interdisciplinary team and families.
  • Work collaboratively with the interdisciplinary team, including physicians, nursing staff, and rehabilitation professionals, to ensure comprehensive care.
  • Educate residents and their families on techniques and strategies to promote functional independence and prevent further injuries.
  • Maintain accurate and current records for each resident under your care, including progress notes and treatment plans.
  • Ensure a safe and hygienic treatment environment.

Personnel Responsibilities

Adhere to work assignments and schedules to complete your tasks efficiently.

Execute all assigned duties in accordance with established policies and procedures, as directed by your supervisor.

Notify your supervisor or Director of Rehabilitation if you need to leave the facility during your scheduled hours.

Report any complaints or grievances expressed by residents.

Collaborate with inter-departmental staff and other facility personnel to ensure therapy services meet residents' needs.

Foster an atmosphere of warmth, personal interest, and positive emphasis, creating a calm environment throughout the unit.

Engage in regular communication with therapy and nursing staff to identify and address areas for improvement.

Professional Development

Participate in scheduled training and educational sessions.

Engage in orientation programs and activities as required.

Attend in-service and continuing education programs as directed, ensuring a minimum of 30 hours of training every two years to maintain licensure.

Health & Safety Compliance

Adhere to established safety regulations within the facility.

Participate in fire safety and disaster preparedness drills in a professional manner.

Report any hazardous conditions or equipment to your supervisor immediately.

Follow established safety protocols while performing all duties.

Wash hands before and after providing any service to residents.

Report any communicable diseases to the Director of Nursing.

Equipment Management

Utilize only the equipment you have been trained to operate safely.

Report any defective equipment to the appropriate personnel.

Communicate equipment and supply needs to the Director of Rehabilitation.

Resident Rights Protection

Maintain confidentiality of all resident information to protect their rights.

Ensure fair and respectful treatment of all residents.

Honor residents' treatment refusal requests and report them to your supervisor.

Qualifications

Educational Background

Bachelor's or Master's degree in Physical Therapy.

Current state licensure as a Physical Therapist is required.

Experience in a skilled nursing facility or with geriatric populations is preferred.

Work Environment

Work in well-lit and ventilated areas, including nursing service areas.

Engage in various physical activities such as sitting, standing, bending, and lifting throughout your working hours.

Interact with residents, staff, visitors, and government personnel under diverse conditions.

Physical and Emotional Requirements

Must be able to lift and push/pull a minimum of 50 pounds.

Ability to stand and walk for extended periods is essential.

Must possess strong communication and interpersonal skills.

Ability to work independently and collaboratively with others is crucial.
