We are seeking a dedicated case manager to join our team in providing essential support to clients in need.

The primary responsibility of the permanent supportive housing case manager is to oversee clients who are beneficiaries of the shelter plus care (S+C) rental assistance program managed by Housing Connect. This program is designed to offer permanent supportive housing vouchers to individuals and families experiencing chronic homelessness and facing disabling conditions.

Key Responsibilities:

  • Assist clients with the eligibility determination process for housing assistance.
  • Facilitate housing searches and placements, ensuring a smooth transition for clients.
  • Conduct comprehensive needs assessments to identify individual client requirements.
  • Develop tailored case plans aimed at promoting housing stability and self-sufficiency.
  • Provide ongoing case management services, including regular home visits and support.
  • Collaborate with various community agencies to connect clients with employment, education, healthcare, and financial resources.
