Patient Care Assistant II

Position Overview
The Patient Care Assistant II plays a vital role in delivering compassionate care to patients under the direct oversight of a Registered Nurse. This position is essential in ensuring a safe and hygienic environment for patients while performing duties with a focus on safety and professionalism. The assistant is expected to maintain clinical and professional competencies tailored to the specific needs of the patient demographic served.
Key Responsibilities
- Aids patients/residents in their daily living activities.
- Provides assistance with personal grooming and hygiene tasks.
- Monitors and records vital signs, including height and weight.
- Supports patients/residents with feeding and mobility in accordance with established protocols.
- Ensures patient/resident rooms are kept clean and organized.
- Observes and communicates any changes in patient/resident conditions.
- Familiarizes patients and their families with the unit and instructs on safety protocols.
- Assists with administrative tasks such as answering phones, entering data, and maintaining medical documentation.
- Manages inventory of medical supplies and equipment.
- Executes duties as specified in the Nurse Aide II scope of practice.
- Conducts technical procedures with appropriate training, such as performing a 12-Lead Electrocardiogram and routine blood draws.
Physical Demands
The role requires physical activities including walking, standing, sitting, lifting, reaching, bending, stretching, stooping, pushing, and pulling. The ability to lift and support weights up to 35 pounds is necessary for handling patients, medical equipment, and supplies. Proficiency in English communication is required, along with manual dexterity. Physical abilities testing is mandatory.
Qualifications and Certifications
- A High School Diploma or GED is mandatory for new hires.
- Must hold a current listing with the Nurse Aide Registry as a Nurse Aide II.
- Basic Life Support (BLS) certification is required as per organizational guidelines.

The candidate should demonstrate an understanding of growth and development principles and possess the skills necessary to cater to the diverse ages, cultures, developmental stages, and unique needs of the patient population.
Confidentiality and Privacy
- Access to protected health information (PHI) will be limited to what is necessary for job performance.
- Information sharing will occur strictly on a need-to-know basis for work-related purposes.
- Access to verbal, written, and electronic PHI is determined by job responsibilities and is controlled through secure user credentials.
Equipment and Tools
The role involves the use of various tools and equipment, including blood pressure monitors, stethoscopes, thermometers, personal protective gear, wheelchairs, stretchers, utility carts, and other relevant clinical instruments.
