Position Overview
The Case Manager plays a vital role in our programs, focusing on assessing and addressing the ongoing requirements of individuals through comprehensive case management strategies. This position emphasizes empowering clients and motivating them to achieve stability, thereby reducing the likelihood of future homelessness. The Case Manager will possess a thorough understanding of local support services for low-income and homeless populations, as well as insights into labor market trends and effective career planning.
The Case Manager will collaborate closely with clients to create realistic goal-oriented plans that prioritize employment, savings, access to community resources, and affordable housing solutions, while diligently tracking their progress to facilitate successful transitions from residential programs.
Key Responsibilities
Direct Client Engagement / Case Management
- Evaluate individual needs and assist in formulating case plans with clear goals and objectives.
- Make suitable referrals to external agencies and services as required.
- Provide ongoing follow-up to ensure completion of referrals, documenting and monitoring residents' progress.
- Enhance communication and coordination among service providers to support client needs. Empower clients to develop problem-solving skills to achieve measurable outcomes.
- Encourage self-advocacy and independence among residents.
Service Documentation and Assessment
- Maintain detailed and accurate records of all case management activities for each participant.
- Document and keep current information regarding services rendered and financial expenditures in the Homeless Management Information System (HMIS).
- Communicate regularly with team members through email and reports, utilizing case conferences and supervision to enhance case management efforts.
Working Environment
Physical requirements include the ability to walk, stand, bend, squat, climb, kneel, and twist on a regular basis. Candidates should be able to lift up to 40 lbs and handle various office equipment.
Minimum Qualifications
- Bachelor's degree in a relevant field such as social work, sociology, or psychology is preferred.
- A minimum of two years of experience in a residential program serving homeless families is preferred.
- At least two years of experience in case management is required.
Skills, Knowledge & Abilities
- Proficiency in providing case management services, including resources and counseling to assist clients in achieving their goals.
- Strong attention to detail with excellent organizational and time management skills.
- Effective oral and written communication skills in English.
- Strong listening and critical thinking abilities.
