Care Transition Coordinator

About the RoleWe are seeking a highly motivated and organized Care Transition Coordinator to join our team at the Harlem United Community AIDS Center Inc. As a key member of our staff, you will play a critical role in ensuring a smooth transition for clients from intake to assigned care management.
Key Responsibilities- Initiate contact with newly enrolled clients via phone within 48 hours of assignment to introduce yourself and initiate the care process.
- Schedule initial face-to-face visits with newly enrolled clients within the week of their assignment to establish rapport and assess their needs.
- Assist clients in obtaining urgently needed services during the intake process, including connecting them with primary care physicians, psychiatrists, cardiologists, oncologists, and physical therapists.
To be successful in this role, you must possess strong organizational and time management skills, as well as excellent communication and interpersonal skills. A bachelor's degree in a related field is preferred, along with relevant experience in healthcare or social services.
