Job Summary

We are seeking a highly experienced and skilled Senior Director of Care Coordination to lead our care coordination programs system-wide. This individual will be responsible for driving the success of care coordination programs, including case management, social work, care transitions, and total care progression.

Key Responsibilities
  • Develop and implement strategic plans to improve clinical and financial outcomes
  • Facilitate transformative care by managing key performance indicators (KPIs) related to health service utilization, quality, expenses, and other critical factors in alignment with value-based care
  • Collaborate with leadership and other departments to facilitate communication and advance the organization's goals and objectives
  • Integrate care coordination into other department workflows through influence, communication, and relationships to support the continuum of care and aid in quality care
  • Utilize data analytics and trending to forecast future needs, programs to improve functional areas with increased patient quality outcomes and reduced costs with efficient processes
  • Develop and establish policies and processes to ensure teams are integrated into the direct patient provider team or other relevant teams as appropriate
  • Recruit, engage, develop, lead, and manage assigned staff
Requirements
  • Master's degree in a relevant field
  • Valid RN license in WI, MO, IL, and/or OK
  • 10 years' experience, with 5 years' in leadership
