KEY RESPONSIBILITIES
  • Conduct comprehensive assessments for clients in community, shelter, and program settings, creating tailored service plans and facilitating appropriate program placements.
  • Coordinate the provision of essential support services, including medical, legal, and ancillary assistance, to eliminate barriers and enhance client success.
  • Establish and maintain partnerships with external agencies for client referrals.
  • Monitor client progress towards service plan objectives, conducting regular reviews and making adjustments as needed.
  • Engage in or lead spiritual activities, such as devotions and prayers, that promote a credible Christian witness and lifestyle.
QUALIFICATIONS
  • A Bachelor's degree in Social Work, Psychology, Counseling, or a related field is preferred.
  • A minimum of 4 years of experience or an equivalent combination of education and experience in case management within similar environments.
  • Proficiency in Spanish is preferred.
  • Experience working with individuals in recovery and those who have experienced trauma.
  • Ability to exercise discretion and maintain confidentiality.
  • Proficient in Microsoft Office applications and familiar with database management.
  • A solid Christian witness and lifestyle that reflects Christ in character and actions.
