Job Opportunity

We are seeking a skilled RN Care Manager to join our team at Sunnyview Acute Rehab Hospital. As a key member of our Case Management team, you will play a vital role in providing exceptional patient care and ensuring seamless transitions for our patients.

Responsibilities
  • Develop and implement effective discharge plans to ensure patients receive the necessary care and support after leaving our hospital.
  • Collaborate with interdisciplinary teams to coordinate patient care and address any concerns or issues that may arise.
  • Provide education and support to patients and their families to promote healthy behaviors and self-care.
  • Participate in departmental meetings and in-services to stay up-to-date on best practices and new developments in the field.
Requirements
  • Associate's degree in Nursing and current RN licensure in New York State required.
  • BS in Nursing or clinical practice preferred.
  • Community health experience and/or CRRN certification recommended.
  • Strong organizational and communication skills, with the ability to work effectively in a fast-paced environment.
