Job Summary

The Clinical Pharmacy Support Specialist (CPSS) collaborates with the value-based ambulatory pharmacist and the multi-disciplinary population health team to deliver services aimed at optimizing patient outcomes in alignment with our value-based contracts. The CPSS engages cross-functionally with various stakeholders to drive data-informed results. This role involves conducting patient outreach through telephone, digital platforms, and other suitable methods to promote successful care gap closure and enhance health outcomes.

Minimum Job Qualifications
  • Licensure or other certifications: Pharmacy Technician Board Certificate registered with the Georgia Board of Pharmacy
  • Educational Requirements: Associates Degree
  • Minimum Experience: 3 or more years of experience as a pharmacy technician with a focus on medication adherence.

  • Exceptional communication skills with patients at all levels, both verbal and written. Ability to engage patients in a friendly and supportive manner.
  • Capacity to build strong, trusting relationships with leadership, physicians, pharmacists, and patients involved in Value-Based Programs
  • Familiarity with medication-related quality metrics relevant to value-based care programs (Medication Adherence - Diabetes, Cholesterol and Hypertension, Statin Use in Persons with Diabetes, and Statins in Patients with Cardiovascular Disease)
  • Highly organized and self-motivated with the ability to adapt to diverse work environments
  • Other professional skills and qualities: strong attention to detail, ability to manage multiple tasks effectively, reliability, empathy, commitment to quality service, goal-oriented

Essential Tasks and Responsibilities
  • The primary responsibility of this role is to support team pharmacists, clinicians, and the care management team in ensuring effective, safe, and economical drug therapy for patients participating in the CIN's value-based contracts.
  • The Clinical Pharmacy Support Specialist works closely with patients to provide comprehensive medication adherence support utilizing various tools, applications, data, and analytics
  • Responsible for gathering medication refill histories, conducting thorough pharmacy benefits investigations, and assisting with provider communications
  • Establish rapport with patients to identify strengths and barriers to medication access, aiming to close care gaps and promote outstanding patient care
  • Develop and maintain relationships with pharmacists, providers, and clinical staff to deliver the highest level of care for patients with complex needs
  • Identify and resolve intricate medication-related issues, knowing when to involve or escalate to pharmacists, providers, or leadership.
  • Facilitate medication reconciliation activities, assist patients in overcoming barriers to medication adherence, conduct medication outreach, and complete community resource referrals
  • Collaborate with local pharmacies to troubleshoot issues affecting medication adherence or access. Follow up with pharmacies to confirm patients pick up medications, closing care gaps
  • Input data into databases, payer portals, and other data repositories to evaluate patient activity/medication adherence rates and address barriers to care to enhance outcomes
  • Review and address medication care gap reports and organize daily outreach lists for patient follow-ups
  • Participate in quality and payer meetings as directed
  • Engage in staff meetings, in-services, and other program-specific tasks and activities as required
