Job Overview

Position Summary

The role of the staff pharmacist encompasses the processing of medication orders for both parenteral and enteral therapies, overseeing patient medication regimens, and providing essential drug information. In the absence of the pharmacy manager, the staff pharmacist may be called upon to fulfill the critical responsibilities and duties typically held by the pharmacy manager.

Key Responsibilities

  1. Prepares and dispenses medication orders as per physician directives, adhering to established protocols and procedures.
  2. Accurately interprets and transcribes medication orders into computerized patient medication profiles, ensuring completeness and accuracy.
  3. Compounds and dispenses pharmaceuticals, including sterile products, chemotherapy agents, and parenteral nutrition solutions with precision.
  4. Manages the issuance of controlled substances to patient care areas, maintaining compliance with legal record-keeping requirements.
  5. Ensures the provision of safe, effective, and economical medication therapies for patients in accordance with established guidelines.
  6. Monitors drug therapy regimens for potential contraindications, interactions, and appropriateness of medication and dosage.
  7. Assists with pharmacokinetics consultations and drug dosing in line with organizational protocols.
  8. Reviews culture and susceptibility data to ensure antibiotic appropriateness and recommends modifications as necessary.
  9. Accurately reads and interprets information from patient charts.
  10. Identifies and reports suspected adverse drug reactions promptly and accurately.
  11. Maintains formulary integrity by minimizing non-formulary procurements and promoting rational drug therapy choices.
  12. Provides clinical consultation and guidance to healthcare practitioners, suggesting cost-effective therapeutic alternatives when appropriate.
  13. Delivers accurate and timely drug information to both professional staff and patients/families.
  14. Documents all clinical activities and interventions comprehensively and accurately.
  15. Engages in quality improvement initiatives and medication use reviews, collecting data and maintaining necessary documentation.
  16. Participates in the development and delivery of training programs for pharmacy, medical, nursing, and other staff.
  17. Maintains current licensure as a pharmacist.
  18. Attends pharmacy staff meetings and participates in ongoing education and training programs.
  19. Operates independently as an Informatics Pharmacist, overseeing the validation of changes to pharmacy modules and ensuring thorough documentation.
  20. Collaborates effectively with physicians, pharmacy staff, and other health informatics teams.
  21. Monitors billing accuracy and compliance, ensuring data integrity through audits and interactions with financial systems.
  22. Facilitates the creation and maintenance of order sets and drug mixtures in accordance with established guidelines.
  23. Educates pharmacy staff on new workflows and training initiatives through various formats.
  24. Completes special projects and additional duties as assigned.

Qualifications

Required Education and Experience

  1. Graduation from an accredited School of Pharmacy and licensure as a registered pharmacist.
  2. Experience with pharmacy information systems is essential.
  3. Current licensure to practice pharmacy in the relevant state.
  4. Ability to function effectively as an inpatient staff pharmacist is required.

Preferred Education and Experience

  1. Experience in a health system setting is preferred.
  2. Familiarity with Meditech systems is advantageous.
  3. Experience in monitoring billing accuracy and compliance is preferred.
  4. Experience in building and maintaining pump libraries is a plus.
