Position Summary
The Clinical Pharmacist is responsible for managing medication orders for both parenteral and enteral therapies, overseeing patient drug treatments, and delivering essential drug information. This role includes supervising pharmacy support staff and may require assuming managerial duties in the absence of the pharmacy manager.
Key Responsibilities

- Processes and dispenses medication orders based on physician requests, adhering to established policies and protocols.
- Accurately interprets and transcribes medication orders into computerized patient medication profiles, ensuring completeness and accuracy.
- Compounds and dispenses pharmaceuticals, including sterile and chemotherapy products, with precision.
- Manages the issuance of controlled substances to patient care areas, maintaining compliance with legal record-keeping requirements.
- Ensures the provision of safe and effective medication therapies in line with established guidelines.
- Monitors drug therapy for potential contraindications, interactions, and appropriateness of medication and dosage.
- Provides pharmacokinetics consultations and drug dosing recommendations as per organizational protocols.
- Evaluates culture and susceptibility data to ensure appropriate antibiotic use and recommends necessary adjustments.
- Accurately reads and interprets patient chart information.
- Identifies and reports suspected adverse drug reactions promptly.
- Maintains formulary integrity by minimizing non-formulary procurements and promoting rational drug therapy.
- Offers clinical consultation to healthcare practitioners, suggesting cost-effective therapeutic alternatives when appropriate.
- Delivers accurate and timely drug information to both professional staff and patients.
- Documents all clinical activities and interventions comprehensively.
- Engages in quality improvement initiatives and medication use reviews, collecting data and maintaining necessary documentation.
- Participates in the development and delivery of training programs for pharmacy and medical staff.
- Maintains active pharmacist licensure.
- Attends pharmacy staff meetings and relevant educational programs.
- Works independently to manage pharmacy module changes, including testing, training, and documentation.
- Collaborates effectively with healthcare teams to ensure positive outcomes.
- Monitors billing accuracy and compliance, ensuring data integrity through audits and investigations.
- Contributes to the creation and maintenance of order sets and drug mixtures according to established guidelines.
- Provides training to pharmacy staff on new processes and workflows.
- Completes special projects and additional duties as assigned.
Required Qualifications
- Graduation from an accredited School of Pharmacy and current licensure as a registered pharmacist.
- Experience with pharmacy information systems is essential.
- Active licensure to practice pharmacy in the relevant state.
- Ability to function effectively as an inpatient staff pharmacist.
Preferred Qualifications
- Experience in a health system setting is preferred.
- Familiarity with Meditech systems is advantageous.
- Experience in monitoring billing accuracy and compliance is preferred.
- Background in building and maintaining pump libraries is a plus.
