About the Role

We are seeking a skilled Occupational Therapist Assistant to join our Home Health team at CenterWell Home Health. As a key member of our interdisciplinary team, you will play a vital role in providing high-quality patient care and supporting our patients in achieving their rehabilitation goals.

Key Responsibilities
  • Assess and evaluate patients' daily living and work-related skills, and develop personalized therapeutic retraining programs to improve their functional abilities.
  • Collaborate with our multidisciplinary team, including physicians, nurses, and other healthcare professionals, to develop and implement comprehensive care plans.
  • Provide direct patient care, including administering and interpreting diagnostic and prognostic tests, and implementing therapeutic interventions to improve patients' strength, coordination, range-of-motion, and function.
  • Conduct regular assessments and evaluations to monitor patients' progress and adjust treatment plans as needed.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination and patient satisfaction.
  • Participate in care coordination and discharge planning activities to ensure a smooth transition of care for our patients.
Requirements
  • Master's degree in Occupational Therapy from an accredited program.
  • Minimum of six months of occupational therapy experience, with a preference for home health experience.
  • Current and unrestricted OT licensure.
  • Current CPR certification.
  • Excellent organizational and communication skills, with the ability to work effectively in a fast-paced environment.
