Job Title: RN - Registered Nurse (Salem)

We are seeking a skilled and compassionate Registered Nurse to join our team at Nuclear Care Partners. As a RN, you will be responsible for providing high-quality patient care in the comfort of their own homes.

Job Summary:

The ideal candidate will possess a strong sense of professionalism, effective interpersonal skills, and a true team player mentality. Previous case management or leadership experience is preferred, but not required.

Responsibilities:
  • Provide routine face-to-face patient visits and staff visits in the home setting
  • Build rapport with patients and their families to implement and manage care
  • Maintain communication with Regional Clinical Director, supervisor, case managers, and other healthcare professionals
  • Coordinate with Clinical Director to implement staff development and maintain documentation compliance
  • Manage care implementation with attention to quality patient care and patient family authorization
  • Perform patient assessments according to physician orders and complete nurse progress notes
  • Provide weekly updates to patients on their applications and educate them on the EEOICPA program
  • Ensure patient and patient-family satisfaction with level of care, plan of care, and care team
  • Perform initial nursing assessments, comprehensive nursing assessments, supervisory visits, and new patient admissions
  • Provide home health aide initial certification training and ongoing education
  • Assist and support HHA's through initial paperwork process
  • Provide clinical guidance and coaching for staff and patient families
  • Participate in home visits in service area per policies and procedures
Qualifications:
  • Bachelor of Science in Nursing preferred
  • Current RN licensure in the state of OR
  • Valid driver's license and good standing
