Job Title: RN - Registered Nurse (Salem)

We are seeking a skilled and compassionate Registered Nurse to join our team at Nuclear Care Partners. As a RN, you will provide high-quality patient care in a home setting, working closely with patients, families, and healthcare teams to ensure the best possible outcomes.

Responsibilities:
  • Conduct face-to-face patient visits and staff visits in the patient's home, providing individualized care and support.
  • Build strong relationships with patients and their families, implementing and managing care plans to meet their unique needs.
  • Maintain effective communication with Regional Clinical Director, supervisors, case managers, and other healthcare professionals to ensure seamless care coordination.
  • Coordinate with Clinical Director to implement staff development and maintain documentation compliance.
  • Manage care implementation with attention to quality patient care and patient family satisfaction.
  • Perform patient assessments, complete nurse progress notes, and provide weekly updates to patients in the Onboarding Process.
  • Develop and implement individualized Plans of Care in conjunction with patients and their families.
  • Ensure patient and family satisfaction with level of care, plan of care, and care team.
  • Provide home health aide initial certification training and ongoing education, as well as clinical guidance and coaching for staff and patient families.
  • Participate in home visits in the service area, per policies and procedures, and as needed.
  • Schedule visits with patients or fill shifts to ensure patient needs are met, making changes to routinely scheduled visits as needed.
Qualifications:
  • Bachelor of Science in Nursing preferred.
  • Current RN licensure in the state of OR.
  • A strong sense of professionalism, effective interpersonal skills, and a true team player.
  • Previous case management or leadership role(s) with experience in interviewing, hiring, onboarding procedures, mentoring, and leading are preferred.
