Job Overview

The Client Eligibility Coordinator operates under general oversight and is tasked with assessing client eligibility, gathering client data, coordinating services with providers, and compiling information and recommendations.

This role is governed by state and federal regulations and agency/institution policies, including the Fair Labor Standards Act.

This position necessitates proficiency in computer keyboarding, frequent travel including some overnight stays, the ability to lift up to 20 pounds, frequent bending and stooping, the capacity to sit for extended periods, including during vehicle travel, overtime, proficiency in utilizing the current phone system, clear articulation, and exceptional verbal and written communication skills.

Job assignments are contingent upon varying caseloads and employee resources as determined by management.

Summary

The Client Eligibility Coordinator is responsible for gathering client information, developing and monitoring case plans, coordinating services with providers, and compiling information and recommendations.

This role is governed by state and federal regulations and agency/institution policies.
Responsibilities

Evaluates service referrals and conducts interviews with applicants and/or family members to clarify eligibility criteria, form completion requirements, and available community resources.

Requests information to assess client eligibility or needs and forwards requests for approval to the appropriate personnel in accordance with established protocols.

Provides information regarding available resources, assistance, and services to applicants and may visit clients and their families in their homes to assess needs and offer family and/or parent behavior modification counseling.

Schedules initial intake meetings with clients and/or family members, following eligibility assessments, to gather written and oral information regarding psychiatric, psychological, medical history, financial status, and other previous professional recommendations.

Evaluates requests and professional assessments, in accordance with policies and procedures, and initiates actions to grant, modify, deny, or terminate eligibility for assistance with recommendations and options for services, referring clients to alternative sources, or preparing additional requests for evaluation and assistance.

Develops individualized care plans and contacts participating private and public service providers for support. Interprets policies and ensures compliance with state and federal laws, regulations, legal mandates, action plans, and policies. May act as a liaison to other divisions and agencies to formulate policies and procedures for community-based programs. Performs other duties as assigned.

Knowledge of federal and state laws, policies, procedures, and guidelines governing client eligibility for assistance programs. Knowledge of community-based programs and services. Ability to interpret and apply policies, procedures, and guidelines in determining eligibility for assistance programs. Ability to evaluate client information, assess client needs, and make referrals. Ability to prepare, present, and review oral and written information and reports. Ability to interview and provide technical assistance. Ability to utilize computer software to generate reports and written communications.
