Position Overview:
This role is essential in assisting families and individuals facing homelessness or those at high risk of losing their homes. The Stability Case Manager will provide tailored support and resources to help clients achieve and maintain stability in their lives.

Key Responsibilities:
  • Identify and engage families seeking long-term support, conducting thorough eligibility assessments.
  • Evaluate client needs, strengths, and available support systems to establish stability objectives.
  • Create personalized service plans aimed at enhancing the well-being of each family member.
  • Provide clients with tools and strategies to tackle challenges such as housing instability, unemployment, and family dynamics.
  • Facilitate connections to Our House services and collaborate with other programs, including Career Services and Mental Health support.
  • Assist clients in navigating the housing system, including conducting housing searches and advocating with landlords.
  • Coordinate healthcare access for clients, ensuring integration of behavioral health and primary care services.
  • Guide clients in accessing community resources such as SNAP and mental health services.
  • Maintain a caseload of approximately 25 families, engaging with them over an extended period.
  • Conduct regular case management meetings to monitor progress and address evolving needs.
  • Ensure accurate documentation and tracking of client outcomes in compliance with grant requirements.
  • Participate in professional development opportunities to enhance case management skills and client engagement.
  • Attend team meetings and contribute to a collaborative work environment that values respect and inclusivity.
  • Perform additional duties as necessary to support the mission of Our House.

Qualifications:
  • A minimum of 2 years of experience in a role supporting high-risk populations.
  • Familiarity with case management practices, counseling, or substance abuse treatment.
  • Knowledge of motivational interviewing and trauma-informed care approaches.
  • A strong commitment to delivering quality services and support.
