About the Role

The Housing Stability Coordinator is essential in delivering holistic support to families confronting homelessness or at risk of it. This role encompasses evaluating needs, formulating service plans, and linking clients to essential resources for housing, employment, education, and overall wellness.

Key Responsibilities

  • Identify families eligible for long-term case management services
  • Evaluate clients' needs and strengths to set stability objectives
  • Create tailored family service plans
  • Provide resources and tools to tackle challenges such as homelessness, unemployment, and family dynamics
  • Connect clients with pertinent services and support programs
  • Assist clients in housing searches and advocate with landlords
  • Coordinate healthcare resources and services for clients
  • Research and refer clients to community resources
  • Advocate for clients' welfare and access to necessary resources
  • Maintain accurate client records and ensure data integrity
  • Assess the effectiveness of services rendered
  • Engage in professional development opportunities
  • Participate in meetings and foster team collaboration
  • Ensure a safe and inclusive environment for all clients

Qualifications

  • Bachelor's degree or higher
