Job Title: Transition Coordinator I

Bridgeway Recovery Services is seeking a qualified Transition Coordinator I - Peer Support Specialist to join our Medically Managed Withdrawal Treatment Team.

Job Summary:

A Transition Coordinator Level I serves as an advocate for patients beginning and during the pre-admissions process and continuing until the patient is discharged from the program. They act as a communicator between clients and healthcare providers to help improve or maintain a high quality of service.

Responsibilities:

  • Provide advocacy and support to patients during the pre-admissions process and throughout their treatment.
  • Communicate effectively with clients and healthcare providers to ensure seamless care coordination.
  • Assist patients in achieving their recovery goals and maintaining a high quality of life.
  • Collaborate with the treatment team to develop and implement individualized treatment plans.
  • Provide education and support to patients and their families on recovery and wellness.

Requirements:

  • One year of relevant work experience in advocacy or peer support services.
  • Current Certified Recovery Mentors certificate (CRM) with the Mental Health & Addiction Certification Board of Oregon (MHACBO) or able to obtain it within six months from hire date.
  • Personal lived experience and successful maintenance of recovery from either substance use issues or both mental health and substance use issues.
  • Ability to pass a pre-employment criminal background check.
