Job Title: Senior Care Coordinator

Husky Senior Care is a leading provider of in-home care services, dedicated to delivering exceptional care to our clients. We are seeking a highly skilled and compassionate Senior Care Coordinator to join our team.

Job Summary:

The Senior Care Coordinator will be responsible for coordinating the delivery of healthcare services to our clients, ensuring their safety, comfort, and well-being. This role requires strong communication and organizational skills, as well as the ability to work independently and as part of a team.

Key Responsibilities:

  • Coordinate and manage the delivery of healthcare services to clients
  • Help manage up to 15 clients and cover shifts for start-up clients as well as emergency shift coverage
  • Manage client expectations towards caregiver and be liaison between the client, caregiver, and headquarters
  • Assist in client assessments and develop care plans
  • Train caregivers, ensuring they are providing high-quality care
  • Support our caregivers by providing support visits, training, advice, and correction as needed
  • Communicate with clients, families, and healthcare professionals to ensure continuity of care
  • Perform regular client check-ins and monitor their progress
  • Ensure compliance with healthcare regulations and agency policies
  • Travel to multiple clients as needed
  • Problem solve in special customer care circumstances, including change in condition, emergency start-up, discharge from hospitals, hospice start-up, and setting up nurse delegation

Requirements:

  • Associates Degree or some college
  • Microsoft Office experience
  • At least 2 years of caregiving experience
  • CNA or HCA license
  • Nurse Delegation certificate
  • Valid Driver's License and willingness to travel to multiple clients in the same day
  • Must be okay with pets
