Home Care LPN Opportunity

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team in providing high-quality home care services to patients in the Southwest Twin Cities Metro area.

Job Summary

This is a full-time position that requires the LPN to work independently in the field, visiting patients in their home environment. The ideal candidate will have a strong clinical background, excellent communication skills, and the ability to work collaboratively with our interdisciplinary team.

Responsibilities
  • Participate in ongoing interdisciplinary assessment of the patient
  • Develop and evaluate the plan of care in partnership with the patient, representative, and caregiver(s)
  • Provide services ordered by the physician as indicated in the plan of care
  • Recognize and report changes in the patient's condition to the RN
  • Counsel and educate the patient and family in meeting healthcare-related needs
Requirements
  • Current license as an LPN in the State of MN
  • 2 years' experience as an LPN
  • Able to travel within the geographic area serviced by the office
  • Valid driver's license, reliable transportation, and proof of auto insurance
  • CPR Certification
