About the Job

We are seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team at CenterWell Home Health. As a Home Health LVN, you will play a vital role in providing high-quality patient care and support to individuals in their homes.

Job Summary

The successful candidate will be responsible for providing professional nursing services under the direction of a Registered Nurse (RN) in compliance with state and federal regulations. This includes providing skilled nursing interventions, teaching, and training activities in accordance with the patient's plan of care.

Responsibilities
  • Provide professional nursing services under the direction of a RN in compliance with state and federal regulations.
  • Develop and implement individualized plans of care to meet the unique needs of each patient.
  • Conduct thorough assessments of patients' physical and emotional needs, and develop plans to address these needs.
  • Collaborate with the RN Supervisor to ensure that patient care is coordinated and effective.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
  • Document patient care and progress in accordance with CenterWell's policies and procedures.
Requirements
  • Graduate of an accredited Licensed Practical Nursing Program or accredited School of Vocational Nursing.
  • Current nursing license in the practicing state.
  • Valid drivers license, auto insurance, and reliable transportation.
  • Current CPR certification.
  • Two years experience as an LPN/LVN in a clinical setting, preferably in a home health or hospice setting.
