Job Title: Licensed Practical Nurse (LPN)

Join our team of dedicated healthcare professionals at Interim HealthCare of the Twin Cities as a Licensed Practical Nurse (LPN) in our Home Care department. As an LPN, you will have the opportunity to work independently, providing high-quality patient care in a home setting.

Key Responsibilities:
  • Conduct thorough assessments of patients' needs and develop individualized care plans
  • Provide skilled nursing services, including wound care, medication management, and vital sign monitoring
  • Collaborate with patients, families, and healthcare teams to ensure seamless care transitions
  • Document patient progress and communicate effectively with healthcare providers
Requirements:
  • Current LPN licensure in the State of Minnesota
  • 2+ years of experience as an LPN in a home care or hospice setting
  • Valid driver's license and reliable transportation
  • CPR certification

Work Environment:

As a Home Care LPN, you will work independently in patients' homes, providing one-on-one care and support. Our office is located in Roseville, MN, and the general territory for this position is the Southwest Twin Cities Metro area.
