About the Role:
We are seeking a skilled Organ Transplant Care Coordinator to join our team at Northwestern Memorial Healthcare. As a key member of our transplant team, you will play a vital role in coordinating patient care across phases, making referrals, and working closely with an interdisciplinary team.
Responsibilities:
- Plan and coordinate comprehensive care for patients undergoing organ transplantation
- Develop and implement personalized patient education programs
- Collaborate with healthcare providers to ensure seamless care transitions
- Make referrals to relevant specialty services as needed
Requirements:
- Bachelor's degree in Nursing (BSN) or related field
- Registered Nurse licensure in Illinois
- 2-3 years of clinical experience in stem cell or organ transplantation
- Specialty certification in transplant nursing preferred
Estimated Salary Range: $65,000 - $85,000 per year
