Job Title: Transplant Nurse Coordinator

The Transplant Nurse Coordinator plays a vital role in the care of patients undergoing organ transplantation. As a member of the transplant team, this nurse is responsible for coordinating patient care throughout all phases of the transplant process.

Key Responsibilities:

  • Provides education to patients and their families on transplant-related topics.
  • Coordinates patient care with the interdisciplinary team.
  • Assists with the consent process and arranges follow-up appointments and testing.
  • Develops and implements individualized care plans.
  • Serves as a liaison between patients, families, and healthcare providers.

Qualifications:

  • Bachelor of Science in Nursing (BSN)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • CPR-Basic Life Support (BLS)
  • Minimum 2-3 years of recent and related clinical experience

Preferred Qualifications:

  • Specialty certification in the clinical area of focus
  • Previous transplant or organ-specific experience
