Job Overview:

We are seeking a skilled Registered Nurse with experience in transplant care to join our team as a Clinical Care Coordinator. This is a challenging and rewarding role that requires strong communication skills and the ability to work collaboratively with the interdisciplinary team.

About the Role:

The Clinical Care Coordinator will be responsible for planning, directing, and providing comprehensive care for patients undergoing organ transplantation. This includes coordinating consent processes, data collection, and follow-up visits, as well as developing and implementing personalized education plans for patients and their families.

Key Responsibilities:
  • Coordinate patient care across multiple settings, ensuring seamless transitions and timely interventions
  • Develop and implement individualized care plans, taking into account patient needs, preferences, and goals
  • Collaborate with the interdisciplinary team to ensure comprehensive care, including medication management, wound care, and nutritional support
  • Provide education and support to patients, families, and caregivers on transplant-related topics, such as immunosuppression, medication adherence, and lifestyle changes
Requirements:
  • Bachelor of Science in Nursing (BSN)
  • Licensed Registered Nurse (RN) in the state of Illinois
  • CPR-Basic Life Support (BLS)
  • Minimum 2-3 years of recent experience in transplant nursing or a related field
Compensation:

$85,000 - $110,000 per year
